Allegiance Home Health in Boca Raton, Florida is proud to announce the addition of Karen Harwood, MSW, CCM to our Care Management Team. With her background and education, Master’s of Social Work, she brings a fresh perspective collaborating with our RN Care Managers. Mrs. Harwood has over 15 years experience in South Florida providing care management services to patients and has been a social worker for Allegiance Home Health the past 2 years. She recently became a Certified Case Manager (CCM) with The Commission for Case Manager Certification (CCMC).

The Commission is a nonprofit, volunteer organization that oversees the process of case manager certification with its CCM® credential. Allegiance Home Health’s Care Management Team manages client wellness and autonomy through advocacy, communication, education, and the identification and facilitation of services. Allegiance helps the client and their family identify appropriate services across the continuum of health care and human services while ensuring that available resources are timely, cost-effective, and efficient.

Mrs. Harwood has provided support to the geriatric population in home health, hospice, and facility settings as a masters level social worker for approximately 15 years. She states, “Evaluating patient and caregiver needs, determining the appropriate plan of care, and implementing needed supports are all critical in ensuring patient safety and improved quality of life. I have been involved with adult children who have the challenge of caring for their mom or dad from long distance. Our Care Management Team at Allegiance bridges the gap for these families and provides peace of mind in knowing that their parents are receiving the appropriate care”.
